Please remember to bring these items with you to your first appointment:
- Completed New Patient Forms (you will receive digital forms prior to your appointment via text or email)

You should also bring any past dental records and/or x-rays, any pertinent medical records, and a list of any medications you are taking (including prescriptions, over the counter medications, and herbal supplements or vitamins.)
It is very important that you let us know if you are on heart medication, aspirin, or anticoagulants, or if you have any conditions such as diabetes, high blood pressure, artificial heart valves or joints, a history of rheumatic fever, or any other serious medical condition.
Tallgrass Dental accepts most major credit cards and insurance plans. We also work with CareCredit to provide a financial option for patients who need extensive work or who don’t have dental insurance.
Our office staff is always available to help you with insurance claims and paperwork. We’ll file claims on your behalf and work with your insurance provider to make sure that your coverage meets your needs and your budget.
Please remember that you are fully responsible for all fees charged by our office regardless of your insurance coverage.
